MHM Services, Inc. in a longstanding partnership with the Massachusetts Dept. of Correction have an outstanding Full-Time opportunity Site Director/Primary Care Physician at Old Colony Correctional Center, Bridgewater, MA, an all-male, medium security facility. MHM is staffed by a mission oriented, multidisciplinary team of physicians, dentists, psychiatrists, NPs, nurses, social workers and other dedicated health care providers all committed to providing comprehensive, high quality care to the underserved. Caseload of 8-15 patients per day. PCP/Site Director is responsible for direct care, supervision of mid-levels, pro-rated share of physician on-call, and rounding responsibilities (3-4 times a year). Hours are 8-4p with availability by phone till 5pm.
The PCP Site Director will work M-F and is responsible for accurate assessment of clinical conditions of both acute and chronic illness by means of detailed histories, physical exam, and interpretation of laboratory data, diagnosis and appropriate treatment planning for male patients/inmates. Additionally, the provider will communicate and collaborate with the members of the multidisciplinary team of professionals.
Other duties include: •Order and coordinate appropriate diagnostic tests and procedures to effectively diagnose and treat health conditions •Counsels patients on preventative care, medical problems and use of prescribed treatment •Establishes treatment plans and insures the continuity of care for patients requiring ongoing health care services •Complies with policies and procedures of MHM and MADOC •Interacts well with nursing staff, other doctors, DOC staff and mental health staff.
